On Site Registration will be conducted Wednesday, June 24th from 6:00 pm to 8:00pm at Lawrence Park (Franklin Road). If you are planning to register but cannot attend, please utilitze our Ongoing Registration program by contacting us via the Contact Page as soon as possible. You can register by email or calling simply by supplying your contact information. Forms and payment can be collected at a later date.SPOTS ARE LIMITED All forms needed are available under the "Important Forms" located on this page.
The cost for registration is $150 *Fees include football equipment (parents will need to purchase black football pants with 7 piece pads, cleats (black preferred), athletic supporter and a colored mouthpiece) You can vistit Webster's Sporting Goods on 5060 E. 62nd Street (317-255-4855) and ask for the "Heat Player Pack" which will incllude all items needed that are not supplied

Must be submitted at time of registration and will be available at registration
- 1: Completed Pop Warner Registration Forms.
- 2: Completed Pop Warner Participant / Consent Form.
- 3: Completed Pop Warner Volunteer Registration Form.
- 4: Photo copy of child's Birth Certificate.
Due by August 1, 2009

5: Completed Med. Release Form with doctor's signature.
(must be signed and dated after 01/01/09 and due 07/01/09) - 6: Copy of current Report Card (will be kept confidential)
Participants must include all required paperwork to date to be placed on a team roster.
Players/Cheerleaders who have not submitted all paperwork will not be allowed to practice and will not be distributed any equipment until required paperwork is submitted.
